Illness scoring systems and predictive models are trained on populations, not individuals. Models are judged based on discrimination (i.e., how well they distinguish survivors from non-survivors), but also on calibration (i.e., how well they perform in different cohorts and risk strata). Just because a score performs well in the population it was developed on, does not mean it will be useful for any given patient in that group (let alone) or a patient in an entirely different group.
